Health care logic? Why haven’t lawmakers sought out price gouger ethics in medical and insurance billing?

Medical billing and care has totally changed in the last 25-30 years.I believe in everyone getting paid,but many charges seem to be way over the top.If health care cost what it should, would insurance be so hard to come by? Or is insurance and health care working together to get everything they can?
